If you look at the ring(that the lens sits into) where it connects to the base block, you will see that it is only held onto the base block by a single screw:

This means that the base block can swivel in relation to the lens mount ring if you're not careful.
If you look at the following photo, you will see that the lens mount ring has a flat base where it attaches to the mount block. Where the flat base of the lens mount ring is, I had a friend mill a 2mm slot, the same width as the lens mount ring, across the width of the base block which means that the lens mount ring now fits into a groove and therefore will not swivel in relation to the base block.

I have not had the chance to take a photo of the base block slot so as you can see more easily.
