Noise Removal

I'm having a problem effectively removing the noise from the photo below.
I have Neat Image 4.4 and Photoshop Elements 3.0. 95% of the time, Neat Image is great at removing tough noise. I used automatic profiling (picking a detail-free spot in the photo) with Neat Image on this photo, but it did a
lousy job removing the noise.

For the photo below, I applied lightening and sharpening but no noise removal. But I can tell you that Neat Image did almost nothing to make it better when I did apply it. Anyone know how to make this noise go away?

I also tried Noise Ninja which did a better job on this photo, but typically has done worse on my other photos. I really don't want to have to buy two noise reducation software apps.
