Before you get started with what will probably be labor intensive, there are a couple of things you can do first. I remember mentioned here before that there are good scratch removers for this sort of thing, some coming with scanners. But I don't recall them nor do I have any. Neat image will decrease a lot of the problem, too. But some time ago I was trying to record a procedure that would help milk everything out of Photoshop's tools as I could. It's not great, but it's simple and will get you as far as below. After getting this far, you will have more source material from which to manually clone or heal. Below the result are the steps as they are written in the action.
